  • NZSE College Pathway Programmes Application Form

    This application form is intended to assist school staff and/or parents in enrolling students into NZSE College's Secondary School Pathways Programme. The information collected is required by the Ministry of Education and other government agencies for statistical and registration purposes. Some questions are mandatory and must be answered. NZSE College will process the enrollment form and notify the schools or send an email confirmation regarding the student's enrollment.     Please note: If applicant is under 18 years of age, the application must be signed by the parent/guardian and all correspondence regarding your application will be forwarded to your education agent and/or to your parent/guardian.

  • Citizenship and Residency

    To qualify as domestic student, you must be a New Zealand citizen (including students from the Cook Islands, Tokelau or Niue who have New Zealand citizenship) OR hold a resident visa, permanent resident of New Zealand OR a permanent resident of Australia residing in New Zealand.
  • Emergency Contact Details (Next of kin)

    A staff member may contact the nominated emergency contact, in relation to the applicant’s wellbeing and safety.

  • School Contact Details

    An NZSE staff member may contact the nominated contact person listed below regarding the student's wellbeing and safety in the following situations: (a) if the student is absent from the programme without prior notice, (b) in the event of a medical emergency, or (c) if there are reasonable grounds to believe that disclosure is necessary to prevent or reduce a serious threat to the student's life or health.

  • Disabilities/Health

    This information is confidential to NZSE College but it could be used by the Student Advisors to obtain further resources and support that will assist you in your studies.
  • Declaration

    Please note that the declarations apply to any future applications by the applicant to any NZSE programmes.
  • Immunisation Declaration

    Please confirm that your immunisations are up to date (Answering dishonestly or inaccurately can endanger the health and wellbeing of others).
  • Criminal Record Declaration

  • NZSE Conducts a compulsory police vetting check for our School of health and School of education students who will be working with vulnerable young children and adults this check will show any PAST, CURRENT and PENDING charges. if you are unsure if you have a conviction its best you speak to your course advisor OR select "YES" and state additional information below.

     

    Answering the question below dishonestly to anything not disclosed may or will lead to NZSE withdrawing you from the programme

NZSE College/ATC may whish to use these images for promotional and other business purposes. I hereby relinquish and waive all my rights relating to the content of the images and their being captured, reproduced, and used (whether or not for profit) on any such media form(s) as long as it associates with the branding of NZSE College/ATC.
  • Device Declaration

    Some NZSE College programmes have been developed for online distance/blended delivery allowing students to complete studies remotely, away from NZSE College campuses. To be able to complete such programmes, you need to have access to the following prior to starting your study: Stable/reliable Internet access from home/work; Access to a laptop/desktop that can connect to the internet with a web browser and a required minimum screen size of 10 inches; Video recording capability (required only for some programmes)
  • Authorisation to contact Employer

    I authorise NZSE College to contact my employer to get industry feedback about my programme of study. I understand this is undertaken to ensure that the programme is fit for industry and the communication with the employer will be limited to academic inquiry only.
