This Application Form is for Domestic Students only.
You must read this Application Guide before filling out the Application Form. The Application Guide and Form contain important terms and conditions of your contract with Bodhi.

Step by step enrolment at Bodhi

1 Fill out and sign the Application Form

2 Bodhi has an enrolment policy which aims to make sure you are studying at a level you are prepared for. Please complete all the relevant sections on the Application Form as directed.

3 If you meet Bodhi entry requirements (if any) or if places on the qualification/courses are limited and if Bodhi selects you for one of those places, you will receive a Bodhi confirmation letter and an invoice for fees payable for the qualification/courses you selected.

4 You must pay all your fees before the first day that teaching starts in the programme of study you are enrolled in, unless Bodhi waives this requirement, otherwise Bodhi reserves the right to cancel your enrolment by letter. If you are paying your fees by Student Loan, apply online at www.studylink.govt.nz or call 0800 88 99 00

Required documents to complete this application
- Two forms of JP certified identity documents: details on page 9 of this form.
- If the names on the IDs do not match, you must also provide a supporting name change document.
- Consent to Police Vetting Form: details on page 10 of this form.
- Entry Test: You must read two articles and write a 600 word essay. Details on page 10 of this form.
- Two Professional Referee Reports: details on page 10 of this form.

Your Documents

You must provide two forms of JP verified identity documents.

One must prove your NZ Citizenship or Permanent Residency (e.g., NZ Passport or NZ Birth Certificate & Driver's License).

Verification of Identity

The person’s identity must be checked by using an electronic identity credential such as RealMe, or sight and copy of original primary and secondary identity documents.

If the names on the IDs do not match, then you must include a supporting name change document.

One form of identification must be primary, and one must be secondary. These must be current and not expired.

8. Fees
If Bodhi confirms your enrolment in writing, you must pay ALL fees (tuition fees as set by Bodhi, student services levies, and course costs) in full by the First Date Teaching Starts unless Bodhi alone decides to waive this requirement.

If any fees are unpaid on the first day of teaching Bodhi reserves the right to cancel your enrolment by letter to you.

Even if you do not participate in the courses, you are still liable to pay all fees to Bodhi, unless Bodhi receives your completed signed Change of Circumstances Form within the relevant Refund Period.

If your loan application is not approved by StudyLink, you are still liable for all fees invoiced to you. You authorise Bodhi direct and accept payment of the relevant tuition fees and Student Services Levy from your student loan account.

10. Acknowledgement & Declaration
I hereby apply for enrolment at Bodhi and I understand and agree to the following:

a) As soon as Bodhi receives my Application Form and proof of my identity, I have a provisional contract to study at Bodhi. I can only withdraw from that contract by Bodhi receiving a signed completed Bodhi Change of Circumstances Form as set out in this Application Form and Guide including Further Terms of your Enrolment at Bodhi.

b) That my enrolment at Bodhi is subject to:

i. Bodhi granting me admission (although in some cases my continued enrolment may be subject to satisfaction of other admission criteria (including the results of Police checks being entirely satisfactory to Bodhi).

ii. Bodhi sending me a Confirmation Letter.

iii. Bodhi receiving payment of all fees payable by me on or before the First Date Teaching Starts* although Bodhi alone may waive this requirement without notice to me.

iv. For some qualifications/courses chosen by Bodhi, Bodhi alone decides that I intend to complete the qualification/ courses I am enrolled in. I understand that Bodhi alone may, by letter to me, end my enrolment if Bodhi is not satisfied that I intend to complete the qualifications/courses I am enrolled in.

c) That if I receive a Bodhi Confirmation Letter I will (subject to my right to withdraw as set out in this Application Form and in the Guide) participate in the qualification/courses I am enrolled in and use my best efforts to successfully complete those qualification/courses.

d) That the information I have given Bodhi is true, and complete in all respects.

e) That I have read and understand the terms of my provisional enrolment at Bodhi, and if confirmed the terms of my enrolment as set out in the Bodhi Application Form and Guide (including Further Terms of your Enrolment at Bodhi).

f) Bodhi can use any photographs/videos and/or recordings taken of me for the purpose of marketing, and media during and after my enrolment at Bodhi. I agree to sign other documents required by Bodhi for this purpose.

g) That I have read, I understand, and I agree to the Application Guide, the Further Terms of your Enrolment at Bodhi and this Application Form.

h) If a guardian signs this form then the guardian, agrees to pay your fees if required by Bodhi. In signing this form the guardian also agrees, on your behalf, to the Further Terms of Enrolment with Bodhi.

i) That all references to Bodhi, including the Further Terms of your Enrolment, were read as references to Te Pūkenga - New Zealand Institute of Skills and Technology, trading as Bodhi.

i) I declare that the total costs of the programme have been provided by Bodhi.

j) I declare there is no material conflicts of interest involving any governing members of Bodhi.
